The purpose of the Facility Search is to
  • Locate, by one or more search items, a facility licensed and inspected by the Department of Health and Senior Services, Long Term Care program.
  • View details about facilities that match search criteria.

  • View details about owners and officers/principles of each facility.

There are 5 ways to search:
  • County and type of facility

  • City and type of facility

  • Facility name and type of facility

  • Facility Owner

  • Facility Officer/Principle

You must select the type of search you want to conduct.

Search by COUNTY and Type of Facility:
All search options are OPTIONAL.
  • Select one of the counties from the pulldown list. If you want a list of all of the facilities in all counties, do not select a county – leave that field blank.

  • Click the check box in front of one or more of the facility types, funding sources and/or specialized care areas.

  • Click on the “Start Search” button to begin your search. To clear a prior search entry, click the “Reset” button and type in a new search.

Search by CITY and Type of Facililty :
All search options are OPTIONAL.
  • Type in all or part of a city name. The partial entry does not need to be the first letters of the name. For example, the entry “orange” will match “Orange”, “East Orange”, “W. Orange” and others.

  • Click the check box in front of one or more of the facility types, funding sources and/or specialized care areas.

  • Click on the “Start Search” button to begin your search. To clear a prior search entry, click the “Reset” button and type in a new search.

Search by FACILITY NAME and Type of Facility:
All search options are OPTIONAL.
  • Type in all or part of the name of the facility. The partial entry does not need to be the first letters of the name. For example, the entry “home” will match “XX Nursing Home”, “YYY Homes” and others.

  • Click the check box in front of one or more of the facility types, funding sources and/or specialized care areas.

  • Click on the “Start Search” button to begin your search. To clear a prior search entry, click the “Reset” button and type in a new search.

To help your search efforts
  • The more specific and complete the search criteria, the shorter the resulting matches.

  • A partial entry does not need to be at the beginning of the facility or city name. Spelling, spacing and punctuation count. If in doubt, use a partial entry.

  • Upper and lower case letters do not affect the search.
